Common causes
Worn or oil-contaminated clutch disc
Insufficient hydraulic engagement pressure (internal leak or weak pump)
Faulty clutch actuator unable to develop the required clamping force
Worn diaphragm spring or deteriorated actuation mechanism
Prolonged thermal overload of the clutch
Incorrect TCM calibration or missing software update
